Substance Misuse Questionnaire

Voices 4 Choices have put together a questionnaire for ex/service users to try to find out what the strengths of the service are so we can build on them and also how service users feel that the services could be improved for example, faster appointment etc.

We can only begin to improve the service and find out what service users want, if you let us know.
